Inpatient Swing Bed Services

The Swing Bed Program is a unique service that allows a Critical Access Hospital (CAH) to provide an extended care option for individuals who have recovered from an illness or injury and no longer need to be in an acute care hospital setting, but are not able to care for themselves at home.

OCH utilizes a physician-led and multi-disciplinary team approach to develop an individualized treatment plan based on each patient’s needs. Our goal is to maximize patient independence and assist patients in returning to their previous level of functioning. The staff provides support and education to help patients and families throughout the rehabilitation and recovery process.

Staff and physicians meet weekly to review patients’ progress toward their goals. Progress reports are provided to patients, family, referring physicians and the referring facility. Patients and families are encourages to attend team meetings associated with the specific patient.
